This form is used to sign-up/register when paying with cash or check.
(credit cards are accepted on-line at www.thenovastudio.com via PayPal)

PLEASE READ THE INSTRUCTIONS BELOW :

REVIEW the class refund policies & info page on our website.
PRINT THIS FORM or if you can't, write the same info on blank paper.
FILL OUT completely, neatly & enclose with payment in sealed envelope.
ATTACH PAYMENT: Please send only the class fee (the materials fee is
separate & payable in cash on the day of class, please bring exact change).
CHECK or MONEY ORDER: To "The Nova Studio" (mailed or in person).
MAIL TO: The Nova Studio 24 W. Richmond Ave. Pt. Richmond, CA 94801
IN PERSON: Cash/Check in envelope & drop through MAIL SLOT at studio.
CASH is accepted in person only at The Nova Studio in Point Richmond.
We CONFIRM registrations via e-mail within 5 biz days of receiving your fee.
NOTE: This form is not necessary when paying via CREDIT CARD on-line.
